• Stage 1: Death of cells which is usually undetectable by plain films

• Stage 2: modulation of the cells which is characterized by localized osteoporosis

• Stage 3: Development of the margin of dead bone which appears sclerotic

• Stage 4: Increase in the margin which takes on a crescent shape

• Stage 5: Is characterized by destruction and maybe collapse of the bone


Causes of Avascular Necrosis


Avascular necrosis can be caused due to numerous reasons. The most common cause of AVN is Trauma. It can happen within eight hours after a traumatic injury. Furthermore, the two most frequent etiologies of atraumatic AVN are alcohol consumption and long-term use of corticosteroids. Extreme alcohol intake may have a toxic impact on osteogenic cells. Also, the use of long-term steroids is connected with 35% of all cases of atraumatic AVN. Corticosteroid-related avascular necrosis is more critical and more likely to affect both hips than noncorticosteroid-related AVN.


Avascular necrosis is also a side effect of other ailments, including pancreatitis, chemotherapy, Gaucher’s disease, hemodialysis, and blood disorders, like sickle cell anemia. According to Ayurveda, the causes like consumption of scanty intake of food, non- unctuous substances, intake of foods which are light and cold by nature, a lack in the lifestyle, indulging in more physical activities and mental strain like constant worrying and fear, and not following the healthy habits of daily routine or dinacharya. Also, these are the causes of AVN;


Rathrijagarana (sleeplessness), Vegasandharana (controlling the natural urges),   Excessive traveling, Traumatic conditions and injuries, Excessive strenuous activities, etc are the contributory elements of dhathukshaya and they result in vataprakopa.


Additionally, the vataprakopa further presents the indication of disease in the weakest dhathu or at the site of more khavaighunyata in the shareera (body).


Avascular Necrosis: Ayurvedic Treatment


The DashamoolKwatha (decoction of ten herbal roots) is given to the patient orally which is said to have Tridosha (three humors) soothing in general and Vata in particular properties and thus producing significant relief in pain. Moreover, KaishoreGuggulu is a drug given to the patient which mainly works on obstacles in the blood vessels. So, this is provided to increase blood flow to the head of the femur.


Panchtiktaka Guggulu Ghrita is also used which is recognized medicine for the treatment of diseases of Asthi and Sandhi (joints) and TiktakaGhrita with milk is especially advised in the form of Basti for these kinds of diseases related to bones. PanchasakarChoorna (powder of 5 sweets) is provided to the patient for KoshtaShuddhi (purification of bowels) and Vatanuloman (normalizing the movement of Vata ).
